Certified Peer Specialist

JOB SUMMARY: The Peer Specialist is responsible for providing person centered planning and delivery of services to individuals assigned in the HCBS Program — Empowerment Services.
DUTIES AND RESPONSIBILITIES:
Provide Empowerment Services and Peer Supports are provided with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms.
Services are delivered in mainly in community and home settings.
Peer Support uses trauma-informed, non-clinical assistance to achieve long term recovery from SUD and Mental Health issues.
Maintains accurate and up-date records in accordance with the programs policies and procedures.
Must be able to travel in the community.
Case conferencing with mental health/Substance Use providers to determine goals/objectives, activities and who will perform needed tasks related to mental health issues for client and family Collaborative and concurrent documentation required

Qualifications

QUALITY IPROVEMENT RESPONSIBILITIES
•Use individual patient data to inform clinical decision-making and identify areas for improvement; use data to track patient progress over time.
•Sharing client data with clients to help facilitate shared-decision making and keep client informed of his/her progress
•Work with entire Bridging Access to Care team to apply a continuous quality improvement approach (i.e. the Plan-Do-Study-Act cycle), to address systemic, organization-wide challenges.
•Complete QI training during onboarding process and participate in QI refresher training as needed.
TRAUMA INFORMED RESPONSIBILITIES
Complete Sections 1-5 of CPI Trauma Informed Approach during onboarding process and participate in Trauma Informed care refresher training as needed.
Understanding the bidirectional relationship among substance use, mental disorders & Trauma
Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions).
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge in trauma-informed, recovery in both SUD and mental health. Knowledge of best practices- Motivational Interviewing, WRAP etc.

$15-17 per hour
