Job Title: Intensive Case Manager

Description: Mental health agency located in Harlem has a full time opening for an Intensive Case Manager for the Blend Case Management Program. Will be responsible for delivering rehabilitative and restorative services to an assigned caseload of consumers with serious and persistent mental illness in scattered site apartments. Caseload of 12 to 15 depending on program status. Total of four (4) monthly face to face contacts per consumer on caseload and as needed. Participates in the Intake and Screening Committee. Will be on call twenty (24) hours for emergency services which may include responding in person. Provide crisis intervention in acute episodes of emotional and physical distress in order to contain consumer’s regression and to ensure safety of consumers. Write individual service plans, treatment plans and bi-annual reviews in a timely fashion for all consumers of the assigned caseload. Ensure that all consumers receive quality clinical services. Ensures that consumers are linked to all necessary ancillary services including psychiatric, medical and dental treatment, vocational and educational resources, and legal and financial entitlements.

Qualifications: Must have a Bachelor degree or higher in Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Physical Therapy, Recreation Therapy, counseling, Community Mental Health, Child and Family Studies, Sociology or Speech and Hearing and at least four (4) years of experience in providing direct service to persons diagnosed with mental illness, mental retardation, alcoholism or substance abuse. Must have excellent interpersonal, communication and writing skills. Computer literacy essential. Bi-lingual Spanish a plus.
