Job Title: Case Manager
Description: Write service plans; make referrals, home visits, entitlements and crisis intervention. Delivers services to an assigned caseload of residents in scattered site apartments. Ensures that consumers are linked to all necessary ancillary services including psychiatric, medical and dental treatment, vocational and educational resources, and legal and financial entitlements.
Qualifications: Bachelor degree with 2 years related work experience. Good writing and communication skills. Bi-lingual and drivers license a plus.
