Description

The Brooklyn Bureau of Community Service is one of Brooklyn’s oldest and largest non-sectarian social services agencies. The Brooklyn Bureau has been a leading provider of social services to families, children, and adults with disabilities since 1866. The Brooklyn Bureau, which presently serves more than 10,000 individuals annually, is committed to a broad and diverse range of services including prevention of foster care placement; crisis intervention; early childhood and after school education; and vocational training, job placement, and clinical services to adults with physical and developmental disabilities, and histories of mental illness.

The Brooklyn Bureau’s Medicaid Service Coordination (MSC) provides assistance to people with developmental disabilities in gaining access to necessary services and supports appropriate to their needs and life goals. It is provided by qualified service coordinators and uses a person-centered process to develop, implement, and maintain an Individualized Service Plan (ISP).

The Brooklyn Bureau’s Enhanced Supported Employment Program (SEMP) provides supports to people with developmental disabilities who are enrolled in Waiver day programming towards achieving greater economic self-sufficiency and fuller participation in the community through competitive employment

Responsibilities: Develop, implement, maintain, and oversee the ISP or Habilitation Plan for a caseload of up to 25 MR/DD consumers. Maintain case records to meet audit standards. Work in collaboration with other service providers to ensure coordination of services.
MSC - Locate resources, programs, and other services for consumers. Assist consumers in maintaining Medicaid and eligibility for services. Have monthly face-to-face meetings with consumers, including quarterly visits to their homes. Hold family and advocacy meetings to ensure service satisfaction. Respond to on call emergencies. Do intakes as required.
SEMP - Gather information to develop a person centered supported employment plan. Ensure that consumers are receiving pre and post-placement interventions and track hours of intervention. Provide work incentive information. Work in collaboration with family members. Document daily and monthly progress.

Qualifications

Requirements: Minimum educational level: A Bachelor’s degree in a health or human service field. Minimum experiential level: One-year experience working with people with a developmental disability, or one-year experience working as a service coordinator with any population. (With a Masters degree in a field of human services, the minimum experiential level is not required.) Good oral and written communication skills. Must be skilled in handling emergencies and urgent matters while staying organized. Must be flexible to do field work and be on call as needed.
Fingerprinting and criminal background check required.
