Description

RESPONSIBILITIES: Perform Case Management for assigned caseload including assistance with housing, timely comprehensive service plans and progress notes. Provide counseling and crisis intervention as needed. Maintain documentation and liaise with other agencies providing services to case load. Monitor compliance and management of the clients’ health and well-being; teaching of ADL skills; advocacy and referral as needed.
