CASE MANAGER – YOUNG ADULT PROGRAM

PSCH Inc. a leader in Health and Human Services with offices throughout the New York Metropolitan area and New Jersey, seeks experienced Case Manager with Mental Health experience for immediate FULL TIME career opportunity in our Supported Housing Young Adults Program located in Brooklyn, New York.

Selected candidates will be responsible for the daily operations of our supported housing program servicing individuals ages 18-21 years old. Coordinating and providing case management services to ensure quality of care to Mental Health consumers. Assisting individuals in maintaining independent living environment. Providing supervision to peer counselors. Facilitating MICA Support groups and assisting in on-going development of MICA resources including day programs, community self-help, twelve-step program and psycho-educational trainings. Assisting consumers in housing/furniture selection process. Facilitating housing subsidies and other private and government entitlements. Maintaining written documentation and assisting in candidate screening and placement.

Qualifications

• Bachelor’s Degree in Social Services preferred.
• Must have Experience working with Mental Health consumers with/without co-occurring addictive disorders.
• Must have a New York State driver’s license, access to a vehicle and be able to commute daily between Elmont, NY and Brooklyn residences.
