Supported Housing Case Manager

The Bridge

Description

The Supported Housing Case Manager will be responsible for providing rehabilitative services to a caseload of 20-25 mentally ill adults living in Level I apartments with the goal of assisting them to move to more independent housing. Duties include, but are not limited to, compliance with requirements including facilitating appropriate admissions/ discharges, maintaining census, documentation & incident reporting; preparing programs for re-certification site visits/audits; conduct ongoing maintenance assessments, and act as liaison with Bridge Maintenance Department and landlords regarding needed repairs; perform all related documentation and administrative duties; attend meetings, conferences and trainings, as required; available for 24-hour crisis management and emergency beeper duty; adhere to all approved Bridge personnel and residential policies and procedures; and carry out job function in accordance with all city, state & federal regulations.

Qualifications

Excellent written and oral communication skills
Masters Degree in Social Work or related field
Minimum of 2 years in a mental health setting
Experience in residential services highly desirable
