Description

Provide care coordination to a caseload of 24 SMI individuals with medical frailties; provide trainings to residential staff specific to the needs of the population, and assist in assuring that appropriate care is being administered; help educate clients and staff about preventive measures to reduce the need for emergency medical intervention; train staff and clients around medication, specifically the various types, potential side effects, proper administration/supervision and possible contra-indications; and conduct health presentations in the community.

Qualifications

License and current registration to practice as a practical nurse in NYS
Minimum 2 years of relevant experience
Excellent oral and written communication skills
