Description

Urban Pathways, Inc. a leader in providing continuum of care services for the homeless and mentally ill adults is currently seeking a Case Manager for our Apartment Treatment Program. The selected candidate will be responsible for providing clinical and case management services, coordinate services, perform crisis intervention, assessment, advocacy and train clients in daily living skills as needed. Maintain charts and ensure quality care to the clients while adhering to agency policies and OMH regulations. Traveling is required.

Qualifications

Bachelor’s Degree and 3 years experience working with Mentally ill, MICA or homeless populations. Bilingual (Spanish) a plus.
