Description

Janian Medical Care, P.C., seeks an Adult Health or Family Health Nurse Practitioner to join our expanding Division of Primary Care. Janian Medical Care provides primary medical care and psychiatric care to homeless and formerly homeless New Yorkers at community based programs including supportive and transitional housing, shelters, and outreach teams. Long recognized as a leader in the development and delivery of on-site psychiatric care, Janian Medical Care has expanded our services to provide high-quality, integrated, comprehensive primary care to our clients. We are seeking a dynamic, collaborative clinician, who is dedicated to working intensively with our complex and marginalized patients, both provide primary care and to help us shape our developing services.

FULL TIME OR PART TIME

The primary care provider will closely collaborate with the referring social services staff to formulate and enact treatment/service plans, as well as coordinate care. In addition, the provider will work with an assigned Medical Assistant or LPN and provide task supervision in order to provide efficient and effective delivery of care. Other preferred skills and responsibilities include: phlebotomy; as well as staff training and consultation.

Qualifications

• Advanced Practice Registered Nurse (APRN), licensed and registered to practice in New York State.
• Must have National Certification in Nurse Practitioner Specialty-Adult/Family Health
• Spanish speaking a plus
• Must have current DEA and BLS certification;
• Must be proficient in electronic health record documentation and Microsoft Office.
