Vice President, Health Homes & Care Coordination

New opportunity at the Vice President- level to lead Health Homes and Care Coordination programs in this time of continued growth.

Reporting to the Senior Vice President for Behavioral and Community Health and working closely with Vice President, Healthcare Fiscal & Business Operations, this senior leadership position will oversee the strategic direction, operations and Profit and Loss for the organization’s Health Homes and Care Coordination programs across NYC and Long Island.

This position will assume full quality assurance and Profit & Loss responsibility to ensure desired performance and financial outcomes; provide supervision and leadership to the management team and establish a plan for providing high quality, integrated care through the organization’s Health Homes and Care Coordination programs. Will oversee the implementation of all new “products” and services, interface with EHR and other technology solutions and coordinate efforts with managed care companies and other third party payers.

Qualifications

In addition to a Master's Degree in Business, Public Administration, Social Work or related field, we are seeking an accomplished Administrator with a proven track record of at least ten (10) years’ in the delivery of community based health care services, of which at least seven (7) were at the senior management level with both clinical and P&L responsibility; experience in the management of Behavioral Health Outpatient Clinics and Health Home operations preferred. The ideal candidate must be comfortable with data driven management and demonstrate the ability to work with providers, managed care organizations and government.
