Nurse Manager, Food Pantry Health Initiative

Community Health Action of S.I.

Description

Develop and oversee day to day and long-term planning for new, innovative project to reduce morbidity and mortality associated with untreated and poorly controlled diabetes and hypertension in low income populations. Project includes community-based assessment, screening, linkage to and retention in care and self-management training. Nurse Manager will: Coordinate and supervise staff and volunteers, Ensure quality delivery of services, Monitor outcomes to evaluate markers and Prepare reports to management and funders. Collaborate with other management and line staff to coordinate services for the clients. Represent the agency to internal and external stakeholders. Direct and develop staff; collaborate with physicians and other providers.

Qualifications

• RN licensure required.
• 3 years program and personnel supervisory experience in a community based setting.
• Knowledge of health care and medical assessments, general medical terminology and
• Strong verbal and written communication skills
• General comfort and ability with electronic health records and Microsoft Office Programs

Salary

Comparable salaries benefit package including medical/dental, 403 (b) & paid vacation/holidays.

Annual Salary: $60's depending on experience
