HEALTH HOMES PROGRAM DIRECTOR

THE BRIDGE

Description

Perform daily operations of Health Home program, clinical, task and administrative supervision of staff; Director will be responsible for solvency of the program including managing and controlling census, overseeing billing functions, active enrollment and outreach services. Program Director should develop proficiency in various Electronic Health Records and Health Home Documentation systems (i.e Care Director, Allscripts, Roster Wrangler, GSIHealth and AWARDS) and be able to provide guidance, leadership, oversight and training to support staff. Represent agency at off-site Health. Home/Care Coordination meetings and workshops, and should remain current on Health Care Coordination issues in an emerging and changing Behavioral Healthcare environment.
Any other duties as assigned.

Qualifications

Licensed Mental Health Professional (LMSW, LCSW, LMHC)
Candidate should have strong communication, interpersonal and analytical skills. Should be computer proficient and have knowledge of Excel and Spreadsheets
