Description
Brooklyn Community PROS (Personalized Recovery Oriented Services) offers rehabilitation, counseling, therapeutic activities, and peer support each day to individuals living with severe mental illness. The program works to help program participants identify meaningful, personal life goals to help them become as self-sufficient as possible. Brooklyn Community PROS has been at the forefront of integrating evidence based practices and recovery into its service model.
Responsibilities: • Develop/maintain relationships with referral sources in the community • Be actively involved in our integration into the Health Homes model of care • Perform intake assessments and write up all related documentation • Train at least one intern per semester on intake procedures • Manage caseload of 5-7 clients; care coordination with outside case management, referrals, vocational assistance, and collateral contacts • Conduct required assessments including Wellness Recovery Action Plans, Psycho-socials, Psychiatric Rehabilitation, Vocational Readiness, Cultural, and Chemical Abuse or Dependence Screenings. • Develop and update Individualized Recovery Plans (based on assessments) in collaboration with each recipient on own caseload. • Facilitate 4-6 groups; design/follow protocols as per PROS service categories • Conduct individual counseling sessions to assist program participants with attainment of the life-role goals specified in their IRPs. • Responsible for maintenance of client records and timely documentation of services as required by Medicaid and the Office of Mental Health. • Document consumers’ movement towards goals in monthly progress notes and modify each participant’s services accordingly. • Maintain and update data in electronic health records as well as OMH CAIRS and NYESS databases. • Provide services according to Evidence Based Practices including Wellness Self Management, Family Psycho-education, Medication Management, Co-occurring Disorders, and Supported Employment. • Coordinate follow-up services once a participant is ready for discharge • Participate in weekly administrative, clinical and utilization review meetings. • Carry a 24 hour support cell phone in rotation with the treatment team. • Report to and assist Clinical Supervisor as needed

Qualifications
Possess or soon eligible for LMSW or LCSW. Previous experience working with adult mentally ill/MICA population and/or vocational services preferred. Knowledge of group facilitation required. Criminal background check and fingerprinting required; screening against the GSA, OIG, and OMIG lists of persons disallowed from working in Medicaid funded programs.
