Assistant Director, Wellness Case Management – Bronx – Immediate Opening

Our WeCARE program is a specialized welfare-to-work program serving individuals with disabilities and helping cash assistance applicants and recipients with complex clinical barriers to employment, including medical and mental health, to obtain employment or federal disabilities benefits. Services include comprehensive medical, social and vocational assessment, wellness, vocational rehabilitation, employment and job placement.

One of the nation's largest and most diversified health and human services organizations is seeking an Assistant Director of Wellness Case Management. The incumbent will assist in overseeing the daily operations and supervise staff teams to develop and implement programs. Ensures case management services are provided to cash assistance applicants and recipients who are engaged in wellness rehabilitation plans to stabilize a medical or psychiatric condition. Also, ensure through quality case practice, continued monitoring, evaluation, staff education and feedback that all services and activities adhere to internal, contractual, and industry-related quality assurance standards and that the program is in compliance with agency policies and contract requirements.

Essential Functions:

• Provides daily programmatic, administrative oversight and direction to supervisory staff involved in Health/Medical and Psychiatric case management programs.

• Maintains current knowledge of trends, changes, and new developments in the field; trains staff on current regulations and requirements.

• Participate in the development and implementation of monitoring tools to ensure target performance goals are met daily.

• Determine the medical appropriateness of determinations and medical and/or mental health conditions for cases.

Qualifications

The ideal candidate will have three (3) years’ experience in the delivery of program/client services and managed care services in a community based or health organization with at least one year at the supervisory level. Program, administrative, or quality assurance/utilization review experience desired. Knowledge of policies and practices of federal, state and city agencies including but not limited to those of the NYC Human Resources Administration (HRA).
