Job Title: Social Worker/Health Specialist (Mental Health Screening Program)

Reports To: Director of Youth Programs

Overview: CASES has partnered with Department of Health and Mental Hygiene and NYC Department of Probation to offer mental health screenings and services to youth receiving Probation Adjustment Services. CASES mental health screening process involves administering clinical screening tools to gauge drug or alcohol use; to assess for depression and to assess for trauma. The Health Specialist administers screenings and is responsible for building a therapeutic alliance with youth and their families. Providing 80% of treatment contacts in youth homes, shelters, and other community locations the specialist supports youth to achieve wellness, recovery and positive relationships in the community. We are looking for a clinician with experience working with youth who are justice-involved and experience in family assessment and intervention, psycho-education and other family support services, including cognitive-behavioral strategies.

Responsibilities:
• Assess program participants using validated assessments to inform individual treatment planning and service delivery; also screen participants for signs of symptoms and exposure to trauma using standardized trauma screening instruments;
• Identify services youth should receive based on assessment of history, current relationships and supports, immediate needs and recommend treatment objectives for the youth’s service plan;
• Provide case management services in the community to support participants to access community resources;
• Provide supportive counseling to address risk factors;
• Collaborate with other staff to provide access to entitlements;
• Link participants to treatment based on their eligibility, preferences and needs and provide escort to community services;
• Act as advocate and liaison for participants in securing access to community treatment, rehabilitation and support services;
• Complete charting using database following program standards and timelines;
• Involve family, and treatment providers in service provision;
• Provide culturally competent services in accordance with CASES policies and practice;
• Develop and maintain strategic alliances with community providers;
• Any other duties as required by the supervisor and directors.

Qualifications

• Master’s Degree in Social Work, Mental Health Counseling or a similar degree and at least two years of experience in the field with one year in a case management capacity;
• Experience serving youth, at-risk youth, out-of-school youth, or youth involved in the justice system preferred;
• Demonstrated ability to engage and motivate youth;
• Thorough understanding of a strengths-based youth development approach to working with young people;
• Experience working with and serving families;
• Extensive knowledge of community treatment, support services, and resources;
• Highly organized and self-motivated;
• Proficient in computers;
• Fluency in Spanish preferred.
