Social Worker PT - Shrub Oak

Description

Phoenix House is seeking Temporary Part Time Licensed Social Workers to join their team in the Shrub Oak Facility.


S/he will be committed to the quality of care of clients and facilitate groups and individual sessions. This will include facilitating educational groups, providing crisis intervention, and conducting individual sessions addressing mental health and substance abuse issues. The Social Worker will assess a client’s treatment needs and provide appropriate care.



** This role is Part-time/Temporary 3 - 6 month assignment**

Qualifications

REQUIREMENTS:

EDUCATION:
• Master’s degree in Social Work (MSW)required
• NYS Licensure (LMSW)--- Licensed Social Worker required


EXPERIENCE:
• Minimum of three (3) years’ experience
• Experience with both substance abuse and mental health disorders
• Experience working with dually-diagnosed clients


KNOWLEDGE/SKILLS/ABILITIES:
• Highly organized and demonstrate effective communication skills
• Computer Proficiency and experience in electronic documentation
• Exercise independent judgment, analyze situations accurately, take effective action, and exhibit follow-through
• Demonstrated understanding of and commitment to the concepts of providing outstanding customer service and care
• Ability to work flexible hours
• Assists staff with level of care determination and referrals as needed
• Conducts interdisciplinary case conference regularly for incoming clients in regards to mental health assessment, medical issues, legal involvement, and behavioral concerns
• Communicates all concerns to direct supervisor
• Works with counselors to ensure that clients receive information/referrals during the admissions process
• Reviews admissions charts and signs level of care determination where a Qualified Health Professional is needed (QHP)
