Case Manager

Description

The Bridge, a prominent behavioral health agency on the Upper West Side, is seeking a Case Manager to Engage adults with Serious Mental Illness (SMI), chronic medical conditions, and / or history of alcoholism and /or substance abuse to assist in identifying strengths, needs, abilities and goals. Must be available for 24-hour crisis management and emergency beeper duty; compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation and incident reporting; timely completion of required documentation for every contact to ensure documentation meets OMH and/or McKinney guidelines for level of care; conduct ongoing maintenance assessments, and act as liaison with Maintenance Department and landlords around repair needs; perform related administrative duties, including correspondence, reports, and record-keeping; attend staff meetings, supervision, conferences and trainings, as required; adhere to all approved Bridge personnel and residential policies and procedures.

Qualifications

B.A. required, MSW or MA in related field preferred.
2+ years in similar field, excellent English/Spanish language skills, both verbal and written; preferred knowledge of AWARDS and teh internet; willingness to learn and participate in trauma informed trainings and interventions.
