POSITION SUMMARY:

Responsible for providing clinical and medication management services to the clients served by the Wellness program.

POSITION DESCRIPTION:
The psychiatric Nurse Practitioner will provide individual and group mental health services to clients served by Wellness, including psychotropic medication management. 100% amount of time will be spent delivering clinical services

•Provides psychiatric assessments and medication management of clients.

•Prescribe and monitor medication.

•Provide case conferencing and psychiatric consultation to Clinic staff and coordinate with Primary Care providers as indicated. Recommend interventions to the care plan team.

•Assists in the development of Wellness Center policies and procedures that ensures and promotes best practices in the care of clients served by the program.

•Participates and presents in the interdisciplinary case conference.

•Ensures program is in compliance with all state, city and licensing regulatory bodies.

•Participates in staff training as required by grant and by agency; provides staff training.

•Document all work as required, including maintaining client medical records consistent with policy and procedure and regulatory requirements.

•Participate in continuous quality improvement (CQI) activities.

•Assist the in promoting good community relations and establishment of community outreach program.

•Participate in various committees and meetings as requested.

•Participate in job training and education programs, meetings and committees.

•Maintains a high level of industry knowledge and inform the Director and medical staff of changes within the field.

•Participates in inter-disciplinary case conferences

Qualifications

JOB COMPETENCY:

•New York State Licensed Nurse Practitioner in Psychiatry.

•Must maintain active Collaborative Agreement with Psychiatrist.

•Two years experience in the mental health treatment of the homeless and dually diagnosed.

•Good written and oral communication skills.

•Able to effectively interact with marginalized communities.

•Able to speak read and write the English language.

•Experience and knowledge of Windows computer systems and programs.

•Knowledge of best practices in treatment of mental health and substance use disorders.

•Must be eligible for clearance through NYS Central Registry for Child Abuse and NYS Justice Center.
