Transition Specialist, Young Adult Apartments at Markus Gardens

Description

Tasks:
• Conducts in-reach off-site and facilitates entry to the Young Adult Apartments.

• Conducts intake screenings.

• Conducts assessments and develops a Housing Transition Plan with all participants.

• Provides both individualized counseling and leads groups focusing on housing readiness for all participants.

• Develops community integration opportunities for program participants, and works with the Senior Resident Advisor to develop social and recreational opportunities.

• Obtains psychosocial histories and conducts functional skills assessments.

• Establishes and maintains liaison with primary provider of psychiatric care.

• Assures access to psychiatric, substance abuse and medical services, and entitlement benefits.

• Monitors, documents and teaches client self-administration of medication, according to program policy and procedures.

• Conducts “critical-time” follow-up upon discharge from the Young Adult Apartments.

• Conducts Life Skills Assessments and develops Wellness Plans with all participants.

• Conducts risk assessments for all participants and develops Individual Prevention Plans, where indicated.

• Liaisons with other treatment providers for purposes of case management/care coordination.

• Leads groups focusing on participant engagement, education, prevention and recovery from addictive disorders.

• Trains participants in safety procedures and conducts fire drills and self-preservation tests.

• Maintains client charts and submits necessary notes and reports, including incident reports, in compliance with agency procedures.

• Facilitates crisis intervention in the event of an emergency.
• Attends trainings as requested by supervisory personnel.

• Serves as “Qualified Mental Health Staff Person.”

• On-call to coordinate crisis management.

• Staffs emergency cell phone on a rotating basis.

• Duties as assigned by supervisor.

Qualifications

Masters degree in psychology or a related field or Bachelors with significant work experience with persons dually diagnosed with mental illness and a co-occurring substance abuse disorder. Experience delivering rehabilitative (restorative) skills to adults with mental illness and transitioning clients to more independent housing

Salary

$40,000 - $41,200 plus generous benefits
