Free Tax Preparation for Working Individuals with Mental Health Disabilities Living in New York City - Two Days Only!

To maximize the value of this service, it is important that you bring the following information:
� Photo ID
� Birth dates for everybody on the return
� Social Security cards for everyone on the return
� W-2 forms for all jobs held in 2008
� 1099 forms for other types of income
� Childcare Provider name, Employer ID or SSN and
amount paid, if claiming childcare credit
� Bank account and routing numbers
� If filing with spouse, he/she must be present.
