Benefits assistance CommentsThe Youth in Transition Program creates opportunities for young adults to become self-sufficient, and experience overall wellness now and into the future. Vocational specialists assist eligible participants to rapidly gain full-time employment, as well as enroll in educational/training programs. A team of experienced staff offers mental health and substance abuse counseling, group treatment and case management services.
Eligibility: Must be 18-25 years old. Have a mental health and/or substance abuse diagnosis, motivation toward abstinence from drugs/alcohol use, level of independence consistent with program goals. The following services are offered. employment, educational & training referrals, mental health counseling, substance abuse counseling, relapse prevention support, anger management groups, housing assistance, and health care referrals. Anyone can make a referral and services are free of charge (benefits not necessary).
