HCBS Peer Support Coordinator

Description

The HCBS Peer Support Coordinator is a New York State Certified Peer Specialist and/or New York State/ OASIS Certified Recovery Peer Advocate with at least two years of experience providing Peer support services/OASIS recovery peer services as well as Supervisory experience of Peer support specialists/OASIS recovery peers. The HCBS Peer Support Coordinator will report to the HCBS program director and will be responsible for effective day- to day delivery of HCBS services to HCBS clients in addition to overseeing the HCBS Peer Specialist workforce. The HCBS Peer Support Coordinator will establish positive, strength-based, inspiring supportive relationships with a variety of HCBS clients.
Key role:
• Provide individualized support and coaching to assigned HCBS clients in a face to face, one to one format (maximum individual caseload 25) and/or to groups (maximum group capacity 16) of individuals based upon the HCBS client’s goals and objectives listed in the Individualized Plan of Care.
• HCBS clients are individuals who have been affected by stigma of a psychiatric diagnosis, substance use challenges and/or trauma who choose and build upon resources / opportunities to attain or sustain meaningful, healthy and satisfying lifestyles.
• Oversight and supervision of the HCBS Peer Specialist workforce, reporting to the HCBS program Director.
Activities will include:
• Conducting HCBS Assessments specific to the Peer service; co-develop the HCBS Individualized Service Plan based upon the goals and objectives listed with the individual’s Plan of Care including reporting on the progress of such goals or need for change of goals listed in the ISP.
• Providing Supervision to HCBS peer specialists that will enhance the skills and knowledge base of providers within the HCBS program.
• Facilitate regular HCBS Peer staff meetings so that they are kept informed of best practice in peer support services, community resources, program development and administrative issues. Facilitate monthly individual supervision with the HCBS Peer staff so that they can provide the best practice in peer support services.
• Co-Coordinate, co-participate in, and schedule activities that are consistent with the expressed needs, interests, and passions of the individuals. Assist in creating and maintaining the safest and comfort ablest environment that offers opportunity for participants to engage and connect with each other.
• Maintain required documentation in a timely manner in accordance with MHAW standards and funding source requirements.

Qualifications

This position is reserved for an individual who identifies as a person with lived experience from haven been given a psychiatric diagnosis, receiving or received mental health services and has the ability to relate to and support others based on the “shared personal experience” paradigm. The successful candidate should possess or be quickly able to acquire the following skills and abilities:
• Qualified for (includes, but not limited to having a high school diploma or GED) and at a minimum, and working toward their New York Peer Specialist Certification through OMH or OASIS Certified Recovery Peer Advocate
• Have at least two years of experience in direct services working with persons who have been given a psychiatric diagnosis and/or trauma survivors
• Have at least two year of Supervising Peer support providers
• Must be calm, patient, empathetic, supportive, have excellent listening skills and the ability to relate positively and enthusiastically to a wide variety of people
• Basic knowledge of the public health/behavioral health system including, but not limited to; mental health services and public entitlements
• Strong writing and computer skills, as electronic record is used. Basic knowledge in computer applications such as Microsoft Word, Excel, Outlook and Social Media.
• Willingness and availability to attend supervision, trainings, conferences, workshops, webinars to enhance their abilities and awareness of best practice
• Clean NY State Driver’s License as some driving with clients will be necessary; must have reliable transportation.
• Bi-lingual, English/Spanish is a plus

$46,000+ (Based on experience)
