The Coalition for Behavioral Health, Inc. in collaboration with the NYC Department of Health & Mental Hygiene have partnered to create a NYC Adult Behavioral Health Home & Community Based Services (HCBS) Provider Network Directory. The directory should be used by Adult BH HCBS recipients, Care Management Agencies, Health Homes, Managed Care Organizations, Government partners, and other stakeholders to:

• Help HCBS recipients make an informed decision when choosing a HCBS provider contracted with their Health & Recovery Plan (HARP)
• Familiarize Care Management agencies and HARP care managers with HCBS providers in order to educate eligible recipients about what individual agencies have to offer
• Help HCBS providers promote their services
• Diversify the HCBS referral pool
